To Apply all Families Must Provide:
Proof of Income for Past 90 days. Social Security card of Applicant or legal document. Proof of residence of each Household Member (SSN, medical card). Picture ID of Head of Household. Proof of Homelessness or eminent loss of housing (eviction notice, fire report, etc.)
For Rent/Mortgage Assistance, Families Must Also Provide:
Subsidized Housing Consent of Disclosure Form. Most recent utility bill (if being evicted ) or Proof of ability to get utilities in their own name (if first months rent). Notarized Eviction Notice (if being evicted). Letter from potential landlord (if first months rent), stating location of residence, monthly rent amount, what is included and not included in rent (garbage, utilities etc) and acknowledgement they will rent to applicant. Most recent Rent or Payment receipt with dates (if being evicted). Outstanding Rental or Mortgage balance (if being evicted). Any other documentation (required for eligibility). Families applying for assistance must turn in all necessary documentation before their application can be processed. Applications are processed on a first come, first served basis once all information is received. Late fees and deposits ARE NOT allowable for assistance.
For Shelter, The Family Must:
Actively work toward self-sufficiency by participating in case management and by contacting various community resources. Families can be sheltered in an area motel for no more than 7 nights. During that time, they must meet with the Housing Case Manager every other day. They are assisted in meeting emergency needs and then must work toward stabilizing their family and securing housing of a more permanent nature.
DMCOC owns two (2) houses, which are used for long term transitional housing. Homeless families, who have demonstrated an ability to work toward self-sufficiency and comply with ongoing case management requirements can be placed in the homes.
SHELTER AND RENT CAN ONLY BE PROVIDED ONCE IN A TWELVE MONTH PERIOD. RENT IS LIMITED TO ONE MONTHS COST PER FAMILY. A MAXIMUM OF $500 CAN BE PAID VIA CHECK MADE OUT TO THE LANDLORD. RENTAL DEPOSITS ARE NOT ELIGIBLE FOR PAYMENT. LATE FEES MUST BE CURRENT AND PAID BY TENANT AT THE TIME OF COMPLETING AN APPLICATION.
