New Horizon Family & Child Development Center

Now have child care opening for the following ages:

• Children ages 16 months to 2 years
• Children 4 years old before 9/1/98
• School Age Children

We are open from 6:30 - 5:30 Monday -Firday

We serve breakfast, lunch, and an afternoon snack daily

School age chidren participate in field trips and service projects

The Following information is needed prior to enrollment:

• Childs Physical within the last 6 months
• Birth Certificate
• TB skin test & Lead Screening (for children 12 months & older)
• Up-to-Date shot record

Fees are based on family size, gross income, and number of children that are receiving child care.
