Please bring all required documentation to your appointment in order to receive help quickly. No applications will be processed and/or approved without all proper documentation.
If you are homebound and/or disabled please call for an appointment over the phone. Once your phone appointment is completed you will be required to bring in documentation before we can process your application. You are also required to download this Universal Signature Page, sign and date it and return with your documents.
